Steroids May Be Overrated in COPD Exacerbation
It has been said in the past that regular treatment with inhaled corticosteroids (better known as steroids) has been shown to reduce the frequency of COPD exacerbation and improve health status in patients with Stage III (severe) to Stage IV (very severe) COPD. A new study published in the February, 2010 volume of Chest however, seems to contradict this theory. After a thorough analysis of 11 studies (8,164 patients), it was revealed that the benefit from inhaled corticosteroids (ICS) was modest at best, and only evident in those patients whose FEV1 was less than 50%. The conclusion -- the benefit of ICS is somewhat overrated and should be reevaluated. It will be interesting to find out if this news will affect COPD treatment guidelines for the future. In the meantime, if you are tak...

LABA Label Changes Apply to Asthma, Not COPD
Last week the FDA announced that, due to safety concerns, there will be some changes in the way that long-acting beta agonists (LABAs) are used in the treatment of asthma. The changes are based on study analysis which showed LABAs were associated with an increased risk of severe asthma exacerbation with related hospitalizations and death in adults and children who used them for the treatment of their asthma. It must be emphasized that while LABAs are used to treat both asthma and COPD, the studies that the FDA reviewed included patients using LABAs for the treatment of asthma, NOT COPD. There is no evidence to conclude that people with COPD who use LABAs are at any greater risk compared to people with COPD who do not use LABAs. It is for this reason that the FDA does not recommend any cha...

Spiriva HandiHaler Approved for the Reduction of COPD Exacerbations
How many of you use Spiriva HandiHaler for long-term maintenance treatment of your COPD? Now, there's is one more reason to continue to do so. Based on data from two clinical trials (UPLIFT and Veteran's Affairs) and over 8,000 patients, Spiriva HandiHaler  (tiotropium oral powder for inhalation) has been approved by the FDA for the reduction of exacerbations in patients with COPD. While the UPLIFT trial failed to demonstrate as it had hoped,  that Spiriva Handihaler could slow lung function decline,  results demonstrated that it sustained improved lung function over 4 years when compared to placebo and reduced COPD exacerbations. If you are currently not using Spiriva HandiHaler for COPD maintenance, now is the time you might want to talk with your doctor about the possibility of addin...
